Elementary and Middle School Enrollment
Enrollment is done at your child's school during the school year; however, during the summer, Phase I of the enrollment process is done at the reception area of the PPS Administration Building, 8111 South Westnedge (just south of the Centre Avenue intersection on the east side).
Parents should bring the following documents when enrolling:
- Proof of residency (such as a renter lease agreement or home purchase agreement, a utility bill with a residential address or voter's registration).
- Certified birth certificate. This document is required for each student and must be presented at time of enrollment.
- Immunization Records / Vision and Hearing (during the summer, this is done as a part of Phase II enrollment, which is conducted upon the re-opening of your child's school). Proof of immunizations is required for all new students and vision and hearing tests are required for young five and kindergarten students. Scroll down for further details on required immunizations.
High School Enrollment
For high school enrollment, make an appointment to schedule classes by calling the counseling office: Portage Northern (323-5475), Portage Central (323-5282), Portage Community High (323-6769).
Immunization information
Beginning with the 2002-2003 school year, schools in Michigan will be required to assess and report the immunization status of all 6th grade students, in addition to the new school entrants, including kindergartners, whom they currently assess.
In the fall of 2002, all kindergartners, 6th graders, and new students must have documentation showing that they have had the following immunizations:
- Complete series of DtaP/Td with one dose in the last 10 years
- Three doses of polio
- Three doses of hepatitis B
- Two doses of MMR
- One dose of varicella (or history of the disease)
According to the Michigan Department of Community Health, this new requirement will help assure that all school-ages children are up-to-date and protected against all vaccine-preventable diseases.
Questions about immunizations can be directed to your school or family physician. Portage Schools recently distributed flyers to K-5 students provided by the Kalamazoo County Human Services Department about the Well Child Clinic.
