Portage Northern Parents Membership Form – This form is for joining PNP as a member. All funds go to support the programs hosted by PNP. Membership does not require volunteering or attendance at meetings. Updated 9/1/2008

Huskie Pride Award – This form is to submit a Huskie Pride nomination for the annual Carl Latora Huskie Pride Award.

Portage Northern Grant Form – This form is required in order to submit a grant request for support through PNP funds. Grant requests are due by the 20th of the month, reviewed by the PNP Executive Committee (generally on the first Monday of the following month) and then put to a vote by the membership at the next regular PNP meeting (generally the second Wednesday of the following month).
