Onsite Sports Physicals. Tomorrow night! 5:30-6:30 at PN. $30 cash/check. Print physical form under Athletics tab on website: portageps.org/o/nhs. Completed physical forms MUST be entered into Final Forms before August 7 (or beginning of athletic season for specific sport). Computer(s) will be on hand tomorrow night at PN to upload your form on the spot or you can upload it remotely at your convenience. No paper forms will be accepted at the school.
